NSW Fitness to Drive Vision Assessment

At Elevate Eyecare, we provide NSW Fitness to Drive vision and eye assessments for drivers who need to meet NSW eyesight requirements.

Whether you're renewing your licence, have a vision or eye condition, or require an assessment for a commercial or heavy vehicle licence, we can assess your vision and complete the required eyesight report.

Who do we help?

Our Fitness to Drive assessments can assist with:

  • Licence renewals requiring an eyesight report

  • Drivers with diabetes or other conditions requiring vision assessment

  • Commercial and passenger transport drivers

  • Heavy vehicle drivers

  • LR, MR, HR, HC and MC licence requirements

  • Drivers who have received a request or form from Service NSW / Transport for NSW
     

NSW requires recent eyesight testing for certain heavy vehicle licence classes, and commercial passenger drivers may also require a commercial-standard eyesight test.

What happens at your appointment?

We assess your vision and eye health against the relevant NSW driving requirements and complete the appropriate eyesight documentation.
 

If your assessment can be submitted electronically, we can securely submit the results to Transport for NSW.

If you have been given a paper form, bring it with you to your appointment.
 

Please bring:

  • Any letter or paperwork you've received from Service NSW or Transport for NSW

  • Your current glasses or contact lenses, if you use them

  • Any relevant eye or medical information

Working with your healthcare team

If your driving assessment relates to a medical condition such as diabetes or an eye condition, we can work alongside your GP and other healthcare professionals where appropriate.

If further eye care or specialist assessment is required, we can also refer you to an ophthalmologist within our professional network.
